Important information about your private billing via PVS HAG
(Art. 12 ff GDPR in conjunction with §§ 32 ff BDSG)
During your treatment, data about you will be collected which must be processed by our practice to the extent necessary to fulfil the treatment contract. This data may also be passed on to third parties (e.g. laboratories, doctors providing further treatment) to the extent necessary, in compliance with the relevant data protection regulations. We will only transfer your personal data to third parties if this is permitted by law or if you have given your consent.
For the billing of our services provided to you, we intend to commission PVS HAG GmbH (PVS HAG for short), a wholly-owned subsidiary of Privatärztliche Verrechnungsstelle Baden-Württemberg eG (PVS BW for short), both located at Bruno-Jacoby-Weg 11, 70597 Stuttgart. The purpose of this collaboration is to relieve our administration of billing issues. This gives us more time to provide optimal care for our patients. Your fees will be billed according to our specifications.
The recipients of the data are PVS HAG and PVS BW. As professional secretaries, both are subject to the provisions of legal confidentiality and data protection, as are we ourselves. We kindly ask you to read the declaration of consent on the reverse side and to give your consent to the described procedure for fee billing, in particular to the transfer of your health data required for this purpose. Your consent is voluntary. Your treatment is independent of this declaration of consent.
The health data processed by PVS HAG and PVS BW will be blocked once it is no longer required for its intended purpose and deleted after the statutory retention periods have expired. You have the right to obtain information about your personal data. You can also request the correction of inaccurate data. In addition, under certain conditions, you have the right to have data deleted, the right to restrict data processing and the right to data portability. Please address your data protection concerns directly to the Group Data Protection Officer of the PVS BW Group (datenschutz@pvs-bw.de).
